Colorado Lab Presents at CHoSEN Forum

We were pleased to present at the spring forum for the Colorado Hospitals Substance Exposed Newborn (CHoSEN) Quality Improvement Collaborative. The forum focused on evidence-based practices in caring for families affected by prenatal substance use and opportunities for innovation in collaborative care. 

Dr. Courtney Everson, Senior Project Director for the Colorado Lab, co-presented a plenary session entitled Piloting a Plan of Safe Care Framework for Colorado: Accelerating Progress and Aligning Investments, along with distinguished panelists:

  • Jade Woodard, Executive Director, Illuminate Colorado;
  • Audrey Reich Loy, Director of Program Operations, and Dr. Amy Robertson, Obstetrics Provider, San Luis Valley Health; and
  • Katrina Baker, Peer Specialist and Community Advocate, San Luis Valley Area Health Education Center.

Dr. Everson led the panel in sharing data-informed learnings from the Plans of Safe Care (POSC) pilot and the Prenatal Substance Use Disorder data linkage project. Panelists addressed opportunities to improve collaborations between community providers and hospital-based teams in the care of families affected by prenatal substance use. Special attention was given to identifying trusted professionals who can connect a family with a POSC prenatally, with the goal of delivering prevention services that can mitigate the need for child welfare involvement. 

“Bringing together implementation, lived expertise, health care, and data perspectives on the panel modeled the type of collaboration that is essential to successfully wrap support around families through POSC,” Dr. Everson remarked. “It was an honor to collaborate with this esteemed group of colleagues to inspire action among health care providers statewide!”
